CAMP MED FORMS

EVERYONE who attends summer camp must have the appropriate medical forms filled out. Anyone who does not have forms filled out will not be allowed to participate in summer camp activities! All medication brought by students to summer camp must be collected prior to arrival and given to camp nurse during check-in along with the appropriate medical forms. Any questions regarding this policy may be directed to the office.
